Your child’s health form can be found on our patient portal. This document is completed by our office and posted after each well visit 7-10 business days after the visit. You will see part II and the immunization page available for you. It can be found in the document area. Please note these forms cannot be accessed via iPhone or iPad. Also ensure you have selected your child in the upper right hand side.
